It should not be difficult to do this. There are, however, a few pieces of information that you will need to pick up to get something working. Emacspeak uses IPC's to communicate with the TTS server -- on posex, emacs passes the TTS protocol into the server proc's stein. I'm not sure how this is done in Windows. The lisp call is something along the lines of "process-send-string"; can look it up if you need a hand. Much of the TTS protocol's finite state machine has been implemented already for the Mac server and can be reused. It's also very much one sided (from emacspeak to server). You can also consider using "accessible_output" found on PyPi rather than SAPI5.
