Hi, 

I've always found that c-u c-e d cap-c does the trick.  Did you do a c-u first?  It makes the change global instead of just your current buffer.

I alas do not have answers for you, but merely a question. Are you using espeak as part of your emacspeak installation? I found the same "capital" issue that you described when using emacspeak with espeak, but I too was unclear as to how to change it.
